Students interested in playing school sports are required to sign up in the school health office.
Students will need a current physical examination and the Interval Health History Form. This form should be handed in within 30 days of each sport season. The physical examination form must be handed in before tryouts/practice starts.
Students who do not follow this process and have not been approved
by the health office will be considered ineligible for sports until they are cleared by nursing staff.
Students must have a physical within 12 months of the beginning of the sport season he/she wishes to play. Students may visit the school nurse’s office to see if they have a valid physical on file. If a physical is on file, students will become eligible for a sport once they return the Interval Health History Form and emergency athletic form with a parents signature—provided all injuries have been cleared by a physician.
Note: All transfers must be approved by Section 2 of the New York State Public High School Athletic Association – View Guidelines
At Catholic Central School, student-athletes are required to submit a Travel/Sports Note when an alternative pick-up arrangement is necessary, specifying the name of the individual, other than a guardian, who will be responsible for the transportation of the athlete. This ensures the safety and accountability of students. Additionally, all student-athletes must sign a sportsmanship pledge, committing to uphold the values of respect, integrity, and fair play, both on and off the field. This pledge is a vital component of our athletic program, emphasizing the importance of character development alongside physical skills.
